Build a customer intake form that reduces back-and-forth

A good intake form helps you estimate the right job, route inquiries quickly, and avoid requests for information you do not need.

Key idea: Ask only for information required for the next step. A shorter, clearer form is more helpful than a long form no one finishes.

1. Decide what you need before quoting

Start by writing the questions you always end up asking by phone. A basic service request usually needs contact details, approximate location, the service requested, timing preference and a short description. For certain jobs you may also need photos or measurements. Keep optional questions truly optional.

2. A sample seven-field intake form

  1. Name: Who should receive the quote?
  2. Reply method: Email or phone, with permission to use it for this inquiry.
  3. Service area: Postal code or neighbourhood before asking for a complete address.
  4. Work type: Choose from 4–6 clear service categories, with an “Other” option.
  5. Scope: One or two plain-language questions about the job.
  6. Timing: Preferred timeframe rather than a guaranteed appointment.
  7. Additional details: Optional notes or photos, with a reminder not to upload sensitive information.

3. Match the form to your actual workflow

For a cleaner, ask about property size and recurring versus one-time work. For a mobile detailer, ask vehicle size and whether interior or exterior service is needed. For a landscaper, ask the type of work and approximate space. Each additional field should answer a question that would otherwise delay your response.

4. Prevent accidental promises

An inquiry confirmation is not a booking confirmation. Set a clear expectation such as “We will review the details and respond with a quote or a follow-up question.” Avoid stating a response time you cannot reliably meet.

5. Use the simplest tool that fits

A basic web form may be enough. For conditional questions, file attachments or integrations, compare dedicated form builders. Jotform is a provider to evaluate; check current pricing, quotas, file retention and privacy controls directly. Use your own test details during setup rather than genuine customer data.

6. Close the loop

Decide where new submissions go, who reviews them and when an inquiry becomes an estimate, appointment or declined request. Test your intake using examples of a complete, incomplete and out-of-service-area request.
